2. Not sure I understand the question, but in the USA, many of the items on your survey are illegal to ask in the workplace. You need to include at the beginning of the survey text a disclaimer that this is voluntary and confidential. For gender, you should add "One positive thing about being male is" and "One difficult thing about being male is" or else those polled can assume that you are seeking to support an unwritten agenda, in which case they might not take the rest of the questions too seriously if they assume the questionnaire hasn't been thought through.
3. In the "real" world of business, these questions would be illegal. So why would you "practice" a questionaire that if illegal? Or do you just have an ax to grind? (You do know what I mean when I say "ax" don't you?).
